  • *All embroidery will be completed as per your account owner’s approved designs. Names will only be embroidered on items that include them in the design.

  • *Once this form is submitted, we will send you a detailed order confirmation and digital invoice.

  • *Orders are not considered final until payment has been made. Any modifications to or cancellation of your order must be done before your invoice is paid.

  • *All custom orders are final sale, please review your order confirmation carefully.
